I don't know where to start but I do have a debug.log that dates back to December 30th 2014 and I'll try attaching it.

I wouldn't mind if the server segfaulted, restarted and somehow RECONNECTED the players. At the moment, players just stare at the inevitable back to lobby countdown, this sucks. It isn't a big problem but does happen with some stock as well as custom campaigns.

Other than blacklisting campaigns to minimize segfaults, is there a valid workaround to reconnecting or figuring out once and for all what the root cause of all the pain is?

You should probably tell us more of what plugins you run, maps/campaigns etc.
You can force reconnect on restart/segfault/crash in csgo and css, so you can do it in l4d2 to "I assume", but naturally loosing progress.

Hmm, do you have anything to back up the statement regarding segmentation faults/crashes?

If the server is running, you still have a way to communicate with the players (such as doing something just "before" a restart), .. however:

I'm just thinking - if the server crashes, how would you actually get it to send the message to the players (to tell them to reconnect)?

This worked fine in css, I have not tried it in l4d2 since I've stopped hosting as much as I used to.
It may work, but it wont remember where in the campaign they where, but might force them to reconnect.

That one is only for manually restarting via server commands "quit" and "_restart", and doesn't do anything to server crashes at all.
