When the elevator or door is broken, does anyone know the reason for the trigger?

My server occasionally encountered this error and couldn't find the problem fixing it.

Not heard of any issue like this. Some plugin causing it? Which map? How often does it happen?

Almost every day.
any map 3-1 ,4-2 ,4-3 ,7-3 ,8-4 ,custom map.

server use tickrate 100.

Are you sure theres a bug and it's not simply something like all survivors were not present in the lift? Because the game doesn't let some elevators go until they're all there.
