Sourcebans not installed correctly?

So I followed the instructions in here https://www.gameconnect.net/projects/sourcebans/manual/ but when I go to [mywebsite]/sourcebans, I get the following error:

Ok, so I figured out it was some files that were missing, so I put them in their respective locations, and we finally got the Sourcebans UI/interface to show up, but we're getting this on the main page:

Any idea why?
