There's an annoying bug that mapgroup votes ( both end match and change map player votes ) change to a random map in the group instead of changing to the winning map, this fixes it ( it's not a perfect fix but as long as no plugins change the cvar "nextlevel" shouldn't have issues.

...what I know, if you have used or in use SM plugin which change SourceMod cvar "sm_nextmap"
or some reason you have map name included in cvar "sm_nextmap" -> SourceMod would force that map to next.
- nextmap.smx
- mapchooser.smx
etc. etc.

*edit This is cvar, it not clear value by itself.

Easy fix is clear cvar in server.cfg (or sourcemod/sourcemod.cfg)
