It could be a race condition between the webserver and the software responsible for the DB backup where the operation to update the thread title is partially complete when the DB backup tanks the connection (and causes php to receive a DB operation failure)

Maybe a FLUSH or something along the lines might fix it, with some type of internal race condition. That was created when the server was overloaded.
