You are using 64 tick settings on a 100 (102) tickrate server.

It's not lagging every time a player loads in, there player model and weapons fade to black.

Oh you mean your skins are turning black? For me it just looked like a map/lighting problem.

For the tickrate add this to your server.cfg

sv_maxrate 0
sv_minrate 196608
sm_cvar sv_maxcmdrate 102.4
sv_mincmdrate 102.4
sv_minupdaterate 102.4
sm_cvar sv_maxupdaterate 102.4
