Why some players get duplicated in Top ?
For example same player is Top 4 and Top 5, or it could be more like one player is Top4,Top5,Top6

The latest versions the author tried to solve this, I am with the latest from GitHub and I have not had this problem anymore. In any case you have the cvar sm_rankme_remove_duplicate, just use it that solves your problem.
