Only bots need .nav files.

Clients don't need it, bots need it. This means you cannot run bots without it (the answer to your question is no btw).

But the bots are controlled by the server..
So technically clients don't need to dl the nav files.. no?

Clients do not need to download the map's .nav file. For some reason, they always try to if the server has one, even though in almost all cases, it's unnecessary; only clients who are interested in hosting the map themselves should care about it, and even then, only if they want to play with bots on it (unless it's an MvM map or something)

You don't really need this extension to fix that, though, as you can copy over a small map's .nav file (or possibly a blank file...?) add it to your download server, then rename it for each map. Clients will unfortunately download your invalid file if they don't already have one, but if they really want to play with bots in singleplayer, they could just generate an actual nav mesh for themselves.

How come no one is fixing this extension??
This is by far one of the most useful extensions/plugins i've seen here (and i checked out quite a lot)

Why is it always the good stuff that gets abandoned

Because this extension is useless.
People commonly see the nav file attempt to download (usually they already have it because its packed into the vpk or bsp anyways), but even if it fails to download, it takes less than .5 seconds to continue on. They just see the last file that was attempted to be downloaded as the map continues to load, and thinks the download is "stuck" or something. In reality, they would take the same time to load anyways.
