Thats code language for "hes behind you". In that case set the damage to zero or, since SDKHooks damage doesnt work with l4d2, heal the friend for the inflicted damage amount.

I like that idea. Sadly, I have no concept of how to go about implementing it. :P
I set "survivor_ff_avoidance 1" so I'm going to see if it helps at all.
