Looks like it has something to do with ClientPrefs.

Do you have ClientPrefs loaded and running without error.

this is probably gonna be a facepalm moment, but what's ClientPrefs? It didn't mention anything about it in the opening post...

The ClientPrefs extension is used to store the clients say sound settings

Players can turn the playback of say sounds on/off and it will be stored for the future (requires SM 1.1)

Which SM version does your server run ?

Again, it sounds like client prefs isn't loaded.

Do you see it listed on the output of
sm exts list
and
sm plugins list
