Are you sure .6 is fine? Perhaps it was .6.1 or the version after .6 that screwed it up. I started experiencing this on the 20th.

I'm trying the legacy version tonight to see what happens.

edit: I mean, if this is the problem what the hell could even be causing it? Like I said looking through the code nothing screams at me saying what could cause it.

Can some one explain more in detail what dirs to have w acess and for the user that runs the server. Clean install no mods.

Thats what device said but who knows what could cause it. It could conflict with another plugin giving the nature of what this plugin does.

I've taken a look at this, and I really can't come up with anything that would effect spawning in the game.

@Criminal: Are you running eventscripts?

Yeah, exactly devicenull, it makes no sense what so ever and I told my players to keep an eye out but nope, they were all fine, and it was usually quite common. Maybe this is an SM bug of some kind that is somehow triggering with TF2 when using rcon_lock. I'll try my best to get some solid answers.

If it helps any....the only server that this problem cropped up on with us is one in which the fast respawn time was built into the map itself.
