Unfortunately this is not a real fix for the problem.
According to John Schoenick from the TF2 dev team, it is a result of an old engine typo that has been exacerbated by recent updates. It began appearing in both TF2 and CSGO around November 2016. The TF2 team fixed it quickly for their game. The issue has been reported to the CSGO dev team via their official and unofficial outlets multiple times, including an on-going discussion on the csgo_servers mailing list, with no response or resolution still.
In most reported experiences, it only happens on custom servers but it has been verified many times that the maps are not actually exceeding or coming even close to exceeding the max allowed entities.
