Server Crashes on Map Change and sometimes randomly HELP!

So I run a Jailbreak server and it seems to crash frequently.

I managed to get 1 crash dump:

If I may ask, whats the last thing you see on your SRCDS after the crash?

If you means the error logs there is literally nothing. And in just normal logs it is the same.
Realized that the crash dumps really don't help much cause we are on windows. But if anyone can help.

I disabled DHooks. Server seems to be handling much better. I will let everyone know if it crashes again. It hasn't yet.

Server just crashed again, same signature and dump file. So its not Dhooks.
