[HELP]Plugin Problem

How to remove this survivor class plugin properly, because i tryed and keeps showing that menu on new round.
It's annoying and the plugin is disabled, it's not working just the menu appears and does nothing.

well a friend has this issue on a server but he disabled the plugin, what it has to do with the sma? :/
