Those dumps seem to indicate that the sourcemod.logic.dll binary is older than what the L4D2 binary expects. I would suggest updating to the latest snapshot and then replacing the SM binary from above.

This probably goes for anyone else still experiencing problems, so I'll edit my post to clear this up.
