Would it be possible to have a plugin ban players that resolve to a certain state/country/city?

Um no? You would have to have a thirdparty plugin installed on the client to resolve the ISP location. And you would need to make an agreement with the client.
Closest thing is GeoIP, and that can be spoofed.

GeoIP only narrows down to a country. GeoIPCity will resolve to a city.

On second thought, it may be a bad idea to automatically ban. Basically I have a guy who keeps coming back after being banned. What about adding IP ranges to a text file and if a connecting client matches any of them, it warns the admins in chat so they can further investigate?
