The issue is probably the databases.cfg file - make sure you've set it up properly. If you want you can post it here and I'll check it (remember to remove your database password first though).

Have it tested again in CS:S and I get no error messages. (SourceMod 1.8.0.5973)
Same in CS:GO no error messages. (SourceMod 1.8.0.6040)

Add this to game/addons/sourcemod/configs/database.cfg

	"signal"
	{
		"driver"			"sqlite"
		"host"				"localhost"
		"database"			"signal"
		"user"				"root"
		"pass"				""
	}

and in game/cfg/sourcemod/signal.cfg change to

signalpm_database "signal"
signalpm_table "signal"

Then it should work!
