The gameserver -> database connection doesn't have anything to do with the webpanel -> gameserver connection. The important part is the gameserver -> database connection to get the bans and admins working. This looks fine in your install!
The webpanel -> gameserver connection is only used for displaying information on the fly or some advanced features like kicking players directly from the gameserver with a click on the webpanel. So it's not crucial for sourcebans to work correctly.
To get that working, you'll have to ask your webhost to allow connections from your gameserver's port (default 27015).

Use this small testing script to get more detailed debugging information. Open the php file and add your gameserver's ip and port as described in the comments.
You should always check beforehand, if your gameserver banned your webserver's ip with listip in rcon. Remove it from the banned_ip.cfg and removeip IP via rcon.
