I basicaly blocked all these types of exploits in L4D/2. You could adapt this script with minimal changes to cover all those types of issues. Classnames and game restriction changes. Also accounting for team 3 not just 2.

Edit: I never saw your version of this before. During the process of writing my version I was asked not to delete the entity because of breaking immersion, that's why I block damage.
