I recently saw this topic and I, too, am having the same problem it seems.

Currently everything is set up (to my knowledge) with my SB; the servers show online, the admin are added, and I can ban players from console that take effect in game. The problem is that all of the bans that are added from the server refuse to show up on SourceBans' dashboard.

I've talked to my host and they have "port 3306 opened for outbound connections," yet it still doesn't want to show up. Also, I checked my logs and am getting absolutely spammed with:

L 06/08/2013 - 00:26:32: [sourcebans.smx] Database failure: [1045]: Access denied for user 'starxxex_sbans'@'localhost' (using password: YES). See FAQ: http://www.sourcebans.net/node/20

Anything I'm missing here?

You need to grant that user access. Try reading the link the error gives you

The user has full access to everything within the database. Is it that SB needs inbound connections open for 3306? If so, my host tells me "no dice!"

Try 127.0.0.1 instead localhost

Unless your server and website are hosted on the same box, you need the static ip of the website instead if localist or 127.0.0.1
