I'd love to see the smoker smoke/gas cloud do damage to the survivors.

Ideas:
Damage over time while exposed / after exposure.

Slowed movement.

Impaired vision (the ol' drug effect?). Boomer impairs vision, so not my favorite option.

Could be triggered off of those scripted vocalize commands (the coughing), a radius around the smoker's death position when one is killed, etc.

I have a feeling that this is something Valve will implement, but you know, Valve time. I think this would provide a good alternative/addition to gimping melee / blocking closets (which many would bitch about). It adds to the game instead of restoring balance by restricting players, and makes the smoker more useful.

As a programmer, it seems to me it could be a fairly simple plugin, though I have no experience with SM plugins.

Try tweaking this existing cvar.

"z_cough_fatigue" = "3000"
game cheat replicated
- Stamina impact of coughing. High number will pin in place for a long time, lower will just slow.

"z_cough_duration" = "5"
game cheat replicated
- How long a cougher continuously coughs for.

"z_cough_cloud_initial_cough_delay" = "0"
game cheat
- You don't cough until smoke cloud exists for this long.

"z_cough_interval" = "20"
game cheat replicated
- How often a cougher can cough.

"z_cough_cloud_radius" = "175"
game cheat
- How far from center of smoke cloud makes you cough.

"z_cough_cloud_expire" = "14"
game cheat

if you adjust the first one it will make them take a bit hit in stamina and make them slower so the hordes or other infected can eat them. If you also adjust how long they cough for it would likely hold them or slow them down for that period of time. Default appears to be 5 seconds.

If you use sm_cvar, yes. They're all cheat cvars.

The cloud damage thing is interesting... but I think it would disrupt the game too much.

- It would displace all the survivors apart, which goes badly against the point of the game. As awesome as this would be for the infected, I think this would be way unfair to survivors.

- It would encourage kamakaze suicide attacks by the smoker, which is supposed to be a ranged player class. Plus, the Boomer is already a suicidal player class and another isn't really needed.

But, that doesn't mean it shouldn't be done! As I said, it'd be interesting to see how this affects the game...
