Do you have any crash dumps? Anything else in the logs that might help to report this to valve?

If it really crashed there has to be some traces left, maybe even in your event logs (given you use Windows as your server OS).

Seems you are using no-steam emulator. Remove it and this player will never be able to connect your server and crash it. And don`t forgot rule in here: "no-steam" = "no support".
