• g_tPrecached trie seems useless. You have sounds cleary defined in the const, there is no way of them being precached two times here.
• In Ham_PlayerTakeDamagePost don't access cvar values in a loop, cache them before.
• Don't use just offset values, use their names. 47 tells nothing to others who would want to read and understand your code. It should be named as m_flNextSecondaryAttack.
• In Forward_EmitSound static is misused.
• Why do you hook Deploy and set m_flNextSecondaryAttack there too ? It should be enough to set only in Ham_Weapon_SecondaryAttack. Probably you should also supercede the hook.
• Remove from your first post the requierement of new ham module, it's not needed.

If you want, you can go on adding knife models, or even making a configuration file from where people could set the sounds and models.
