For quite some time I've had this flickering issue on the server.
Players online doesn't seem to affect it, happens no matter the amount.
Using CKSurf 1.8f, if that can have anything to do with it?

The flickering happens with everyone on the server.
It gets sorted after changing the map (to the same one).
Happens usually about 3 times a day.

Any idea what could cause this?

You already know that the cksurf (1.8f) you are using is very outdated and has problems in it that haven't been fixed yet or it is unstable and may have the problem you report here...here are 2 links, which are relatively up-to-date, but please make an backup before (when upgrading to surftimer it was not possible for me to enter the points correctly when upgrading or there were errors, I had to do a fullwipe here)

Indeed, quite familiar with the issues to say the least.
Though I don't get how the surf plugin could make the screen flicker for every client?
Never seen any other CkSurf server with the same issue either.

Maybe the "other" server are also not using the outdated version that you are using, the version you are using has several problems and Niko has not been working on the timer for a long time, so I would advise you to switch to the two (current is surftimer updated, but also has its problems). Fluffy also wrote a post in the Discord that it is not really worth updating SurfTimer because it has bugs.

So currently it means to wait, maybe one can create a new timer that runs without problems or supports the SurfTimer 100% and fixes the current problems. One of my friends (Bara) had recently started to create a updated Zones Plugin: (click me). There would be missing the timer and the replays, let's see what else will happen over the years but I would really advise you to leaven from Niko his version and use one of the other two plugins.
