My first thought as well, however I'm not seeing anything in the code that is doing shady stuff. Closest i've seen is using TF2_RemoveWearable but not killing the entity.
Plus this doesn't happen in a test server running the gamemode. Going to look through the difference of plugin lists.
Happens sometimes on our trade/idle server aswell.
I was under the impression the server had no real persuasion over what the client has in their loadout settings?
We've had issues with the item server every once and a while, nothing a reconnect doesn't fix. But also this happens and you can immediately reequip the cosmetics if you catch it.
Also, we don't run any plugins that give artificial cosmetics to players, the closest we do is remove them, change their iTeamNum or making them invisible.
