Anyone have any ideas on why sometimes some blocks disappear after they've been altered?

Do you have any way of recreating it? I've taken a look and only thing I can think of ATM is that it's using entity indexes instead of entity references. You can try this edited version to see if that fixes it.

hmmmm, I've had your version running for a few days now with no complaints. I haven't had much time on the computer so the most I was able to do is load it and make sure the plugin still did what it was suppose to do. But, from talking with admins and users who are in there a lot, it sounds like the issue is gone. I honestly have no way to recreate it. I think it may be happening when going from a map that needed it to a map that didn't or some weird combination like that. I did forget to mention its always 2 platforms. Thank you for tweaking it.

I have the same problem with the things not showing always on reload they show up. i have updated this plugin and it still dont show.
is this still happening to you or just only me now?
