I apologize for repeating

The problem occur's when one of the players (CT) after dying , watch others playing or leaves game while bomb is planted
It appears that even after whole (CT) crew is dead you have to wait until countdown is over to finish the round
On (T) side you need to wait to the end of round time
It happens not all the time , just need to point that such problem exist.

Plugin can cause this error ?

Only way I can see finding a fix is to play on the server yourself and find out how to get the problem to occur every time and then just disable each custom mod one by one until it starts working properly.

I would first test BombSite Lock (0.1) by naXe as that appears to modify the behaviour of bomb placemat.
