What SB & SM version do you have?
I have SB 2.0 and works fine after the update.

I'm gonna bet you updated SourceMod during the last week and overwrote your old databases.cfg file with the default one from the new SourceMod install by mistake. There is no SourceBans section in the default file and you're settings are gone. See if you have a backup of the databases.cfg file or copy the one from one of your working servers over and replace the default one that's in there now.
