off topic:
oh lol you are this person with that cute voice xD ?

can't you see in the log files who has given admin rights to a person ?
here I think:
Code:
http://YOUR-BANS-SITE . com/index.php?p=admin&c=settings#^2
oh I see you already checked:
"When I checked her account under SourceBans, she still had Server Y un-checked for Server Access."

if the owner of server x has acces to server y ftp then he could probably add in simple-admins.ini
check that file if there is anything written.

btw if admins would abuse their power I would take them away after a warning.
Already did this and everyone is happy.

Do you mean admins_simple.ini, or a different file? Because the first things I checked server-side were admins_simple.ini and admins.cfg under addons/sourcemod/configs and she is not listed in there. Could any other file(s) be overriding SourceBans and giving her Admin powers?

That is what I do too, but this is a unique situation. Keep in mind that I tried to make this work for a year already with Server X before creating Server Y; so there were plenty of warnings. If it was so simple as removing this Admin from all servers, I would have done so already.

I have no issues with Admin Abuse on any of my other servers (or from any of my other Admins) with Server X being the exception (and now Server Y due to this situation). I keep a pretty tight ship on this sort of thing otherwise.

How is admins server permission set up in your sourcebans?
do they have permissions for different servers? or have you set up a server group where all servers are in?

Admin abuse is always a valid reason for a swift perm ban ;3.

Under "Server Access" there is an "All" and "TF2" setting so that if new servers are ever added, I don't have to individually update every Admin to access them. When the Owner of Server X gave this Admin her powers on Server Y again via SourceBans, they sneakily changed her access to "All" hoping that I would not notice. When I removed his ability to edit or add Admins, I made sure that this was once again un-checked on her account. Server X is currently the only server checked under Admin Server Access for her account.

These are the Server Admin Permissions for her account, but again, access to Server Y is un-checked:

The flags are to just give her access to a character model menu since the Owner of Server X made a special model just for her to use on both servers. Donators have the exact same flags and can access the same menu. I also tested an Admin account with these settings/flags and it wasn't able to access Admin on the server.

Could these permissions be giving her Admin on the server even if it is NOT checked under Admin Server Access?

I don't know what to do at this stage other than to grep for her SteamID on the machine. Are there any other files besides admins_simple.ini and admins.cfg within SourceMod that he could have edited to give her Admin server-side outside of SourceBans?

try using sm_dump_admcache in the server console, and look if she is indeed admin on the server..
did you say the owner of server X had created some mods for Server X that is also on Server Y?

Because then he could have made a "backdoor" in one of the mods, that adds her as admin everytime she joins the server.

could be something like this
