I have seen this bug since the time valve patched the game on the 18th of october from memory.
Had a smoker charging a player who was in ghost mode, couldn't move and also some other instances similiar. The only way to fix it was to spawn a boomer next to the person being "charged" and slay or shoot it.
My servers are up to date FYI - i know after valve "fixed it" the charger issue was gone but recently it has re occured on a few occasions.

- Tweaked boss zombie spawn timing in Versus to prevent the Tank spawning as soon as the Survivors exit the safe room.
- Fixed the first maps in Dead Center and Swamp Fever not using the boss spawning rules for starting maps, which are more restrictive than other maps.
