My Plugins are:

SouceMod 1.2.3
MetaMod 1.7.1
Zombie Havok
Unrest 1.0.3
Force Mission Changer (don't know what version but the most up to date one)
left4downtown 0.3.1.0 (not plugin, but thought I'd mention it)

And I think that's it, I have the nextmap.smx in the disabled folder because im using ForceMissionChanger.

I edited out the Password, is it still showing?

Try taking out the FMC plugin and see what happens. I faintly remember reading about that issue in that plugin's thread. No, the password is gone now.

I removed FMC before and just ran your plugin, but still it looped on the first map. But sometimes if I skip the first map, it will go threw the campaign. But not all the time...

Have you looked through both the server logs and the SM logs?

For future reference please attach logs as text files . Much easier to read and for users to browser the topic. The best thing I can suggest is take out all your plugins and add them back one by one.
