read the first post or the .sma to find the cvars.
in your amxx.cfg add this:

amx_parachute @all

how i can put the "parachute model" file ???

the default model is located in: cstrike/models/

you want to use another model ? just edit the sma and put it where you want, for example, this parachute is located in a folder called: cstrike/models/paintball/.
if you want to use it then put it there.
