Yep, you should create a new one.

If you host a dedicated server with srcds.exe, you can set those valves in server.cfg. However, if you host a local server(listen server), you should set them in listenserver.cfg, this file is available in your left4dead2\cfg folder.

Everything works perfectly, I presume. But for some reason there were never more than 4 people ingame. When it reached that amount, I never saw another player joining through quick match. When someone left, somebody else joined, but never a 5th, 6th, 7th or 8th player.

Any way to prevent that?

Try through 'Join Steam Group'

That's not what I meant.

When I create a public campaign lobby, pick Nick and hit start immediatly, I see 3 people joining ingame. Now there are 4 people playing and 4 BOTS running. But that's it. Never do the other 4 slots get filled.

With the new version, I have to manually spawn the extra bots, up to the
max limit. But they all don't spawn correctly, leaving me with without the extra firepower I need to start through the maps. The ones that don't spawn
do the blinking thing and never fully materialize. Here's the problem:
The last few players to join will spawn at the safe room, which is then miles
from where the rest of us are. You can understand the problems this causes.

EdIt: I found an older post/version of this plugin at the L4DToolZ thread.

This version has all the survivors right there.

Mplayer1.4.4 is a primitive one of multiplayer,it works stable and perfect. But it has less feature of l4d2.
Temporarily,You can fill with bots at the begin of game.
Here is multiplayer0.9.4beta, plz give me a feedback.

version 0.9.3 report:
The cvar double_item_on which set in server.cfg didn't work.
I disabled this option in server.cfg.
(double_item_on 0)
But there was still double items. Typing it in console manually was worked, but changed to next map, it reloaded server.cfig setting again.

Let's try version 0.9.4 beta firstly.
-------AFTER 0.9.4 beta TESTING--------
"double_item_on 0" or "sm_cvar double_item_on 0" are unworkable setting in server.cfg.
And it has the same problem like version 0.9.3, if less four players (for example, three human players) in game, once all of survivors died, restart level would left only three human players, the only one bot or reminder would be kicked automatically, but it's not necessary.
