Hi there . I have tryed and look all over!
I got source bans working fine on webside conects to my server i even can ban and so on but when in game i dont seem to be work i have set it up rite, well i think so

my sourcebans.cfg i have server id set to 1 and autoad server to 1 DatabasePrefix" "sb"

I am getting this in my logfile .

L 08/31/2009 - 14:22:22: [sourcebans.smx] plugins/basebans.smx was unloaded and moved to plugins/disabled/basebans.smx
L 08/31/2009 - 14:22:22: [sourcebans.smx] Database failure: [2013]: Lost connection to MySQL server during query
L 08/31/2009 - 14:28:09: [sourcebans.smx] Database failure: [2013]: Lost connection to MySQL server during query

Some help would be great thank you

Well, only thing I can think of is using port 21?

Maybe try and change the port to a default mysql one?

Maybe that is why it looses connection?

Why bans are not expiring on time ? many of my players were complaining that their bans are not removed from the DB after the ban time is finished ,and i checked this myself today and it's true ... why is that happening ?

Because SourceBans uses a 5 minute temporary ban to block people from connection spamming, but Valve's ban system sucks so it regularly doesn't remove the ban after those 5 minutes.

Because Valve's ban system sucks, and those 5 minute bans are not actually 5 minutes long.... yes, it can last much longer.

Generally until the next map change, or server restart.

But the bans should show up as expired ont he website. It's just the players aren't able to join the server immediately caused by the above problem.

Normally SourceBans removes that temporary ban if the player had been blocked within the last 5 minutes or the ban was added <= 5 minutes ago.

It would be nice if you guys added a feature to require a login to view sourcebans opposed it being public and google view.

use .htaccess and .htpasswd, it is what we do:
