Hey found the reason of the error
Now going to get it to the final release.
So I got it working !
Anybody interested ?

Just report the first post in this thread with something like "Errors reported for a couple months now and no fix from the plugin author."

Hopefully staff will toss it into the unapproved bin so folks can stop wasting their time with it.

Is it also possible to make somekind of PHP website, where we can see a list of the players who have warnings, name, steamid, who warned and how many warnings they have, on a website?

Seems to be broken after the latest valve update. Sometimes the plugin cant connect to the database. All my other plugins such as Sourcebans, and serverlogs doesen't have problems to connect the Mysql database.
