The other thing I noticed is I sped up the bot generation on map load. It could be a timing issue, bots are spawned too soon and fail. I will add some error handling to track if this is the issue.

The 'can't get back in' issue is related to the player max, the server reports it is over 4, which is the max for co-op. The signatures posted above may address this and allow people to join matches in progress.

One of the issues when using scripts that the plugin addressed was that map changes had to be "rcon'd" E.g. "rcon changelevel l4d_hospital01_apartment"

After choosing a stock map, you should be able to rcon to a custom map. As long as it isn't a cersus map and has "01_" in the name, the bots will respawn and allow spectators to switch.
