It is about versus server where players can choose the team themselves. I mention that because in The last Stand, the bots stay neatly where they are during the timer, there is NO FENCE, but if you switch to the Survivor team as a SI player, you can still walk away during the timer.
This is the Ready Up plugin that I wrote, which does everything you're looking for and provides numerous customization options. I wrote it because all of the other "Ready Up" sort of plugins are poorly-written or missing necessary features.

This plugin locks all doors until everyone has readied up, has an optional timeout period, and freezes players in place (while disabling commons & special spawns) during pre-game on first maps of campaigns. It also provides dozens of IPC calls for developers who are looking to expand on its functionality.
