So I've been experiencing issues with my admins and VIP (flag A for my servers) where we don't have our access for Admin or VIP commands for several rounds in. This is what I'm currently running in my setup and I have no errors when I did this list via rcon in CP.

What do you guys think could be causing it?

you have both, sourcebans and Sourcemod SQL admins + admin manager ? why ?

hmmm good question. Do you think I should try removing one of these at a time to see if it helps anything?
I don't see why we have them all. We do run a VIP setup and utilize Sourcebans for our admins and linking them to each one of our servers.

You could try move those SM sql plugins in disabled folder and unload from sm plugins list.
But I'm not sure does this fix your problem.

So I removed that plugin and still have the same issue :/ Anything else you think it could be by any chance? If not its okay ill just have to try and isolate the issue myself
