Depends on the execution time, whether Sourcebans gives the flags after or before admins-flatfile.
It also depends on whether you have that user in Sourcebans.

Honestly, if you're using Sourcebans, just enable backup, it fills out all this for you and in case mysql goes down it uses the backup files. Which makes admins_simple.ini, again, irrelevant because it would just need to read admins.cfg.

If for some odd freaking reason, you have to have someone in admins_simple, don't put them in sourcebans. Then you wont have a problem.

This works. My god.

Ok I deleted the person from sourcebans, and kept them in the admins_simple.ini and they have admin access.

Now here's the ticker: Why is admins.cfg and admin_groups.cfg not being loaded?
I dont understand why sourcebans is not putting those flags in properly.

It can't be a database issue.
It can't be a rcon or password issue.
I've got a copy of every config file ever inside and outside of everything, edited and applied accordingly.

what am I doing wrong here?!

What version of Sourcebans are you using, is it the fork 1.5+ or 1.4.11?
Either way, make sure backup is enabled in the sourcebans.cfg and that you have restarted your server.
Check the error logs for any errors.
Make sure you have the correct ID set for all the servers, and that it corresponds with Sourcebans.
Finally, make sure rcon is correct in Sourcebans.

Use "sm_dump_admcache" to inspect the real state. I'm gonna move this thread to the SB forum as the issue is there and we are quite frankly fed up with the support burden it is causing.

Using sourcebans 1.5.2F with modded admin-flatfile_fix.smx installed.

Literally the only thing that was stopping sourcebans from working was that the admins configured from the start needed to be deleted, then reloaded into sourcebans.

But you know what, I suspect it was when I switched over from 1.4.1 to 1.5.2F
The admins from previous sourcebans version were put into 1.5.2F

Solution: delete, then re-add admins.
(assuming that you have set up everything else correctly)
