Metamod basically needs a list of all of the game's entities to work. When a new version of metamod comes out that "supports" a new mod or a new version of an existing mod, it basically means they added or upgraded that list of entities manually.

However, that version of metamod searches through the game's dll to retrieve the list of entities (there is a program that can output all of the entities to a text file, so there is no reason a server-side addon can't do it).

AMXx has nothing to do with this. But, yes, it's possible to have a no-update version of AMXx. Just... don't upgrade it. Like 1.01 users.

Metamod-p actually combines metamod and the gamedll's import tables together, effectively making them equivalent in the eyes of the engine (fast entity linking with no wrappers involved).

As per Avalanche, I would hope there are no 1.01 users. There were important security fixes and vital optimization changes.
