The catch is that the game is currently bugged and fires OnGiveNamedItem for the second Misc slot every time you touch a resupply cabinet.
Every spawn and every time you touch the cabinet, so I guess that is the same time as TF2Items_OnGiveNamedItem. I always just use that event as I find that "player_spawn" can be unreliable.

It might get called twice then, I'm not sure. It's never caused me any trouble, but you could easily get around it by only letting it get called if it has been longer than 0.1 seconds since the last time it was called for that client.
