To my knowledge, you can set the csm plugin to clone mode and when you select Bill it works without crashing.
1. I use l4dcsm_a

2. Bill works without crashing on csm_a as the HUMAN player to my continued knowledge.

As a human player

- Bill will switch back to Nick on a map change to prevent the crash
- Then he will switch back immediately to Bill on a new map
- Sometimes causes slight delays in hordes/or longer hordes due to Mii's work around
- Hands will flash periodically as part of Mii's work around

Now Bill as a bot in csm_a

- Works vocals, icons and all including all dialogue with other L4D1 survivors and/or L4D2 survivors with custom talker files

- WILL NOT switch back to Nick on a map change thus crashing the server, in earlier versions Mii coded Bill to switch back to Nick on a map change but for whatever reason never kept it with his final version.

I believe if this was restored and Bill switched back to Nick on a map change and then switched back to Bill on a new map. This might solve that long lasting crashing issue

Also on a side note.

Though it would probably be more tedious work. The Passing as it has in the past will glitch the L4D1 survivors into their usual spots as bots in the campaign.

Two solutions

1. Workaround to keep them from doing that
2. Keep them model only on the Passing
