No it doesn't.. That only happens if you don't specify the Menu Key bit to use..
For example, many plugins determine if a menu option should be disabled or not by methods like is_user_alive() and if the user is not alive, then they just display the name in the menu with a grey color (\d) but dont add the associating menu key bit...
Crude Example:
1. Username1 <--- guy is alive so add Menu Key Bit 1 = (1<<0)
2. Username2 <--- guy is dead, make name grey. Dont add menu key bit
3. Username3 <--- guy is alive so add Menu Key Bit 3 = (1<<2)
0. Exit
Your menu keys would be: (1<<0 | 1<<3) which means Menu Options 1 & 3 are enabled, but option 2 is not..
