The word order in your translation is wrong. In german you have subject - predicate - object. The word "Name" is an object in this case. %s1 is the placeholder for the subject and "geändert" is the predicate. Sadly in the wrong time-form. Here past perfect while it should be simple past tense and it is misssing the adverb like "hat geändert" (had changed) or "wurde geändert" (was changed).
