The purpose of this guide is to attempt to provide a comprehensive guide to server hosters who are unsure of what actions could result in their server getting banned.
In addition it should be noted that it is never a good idea to use tokens for a server from an account which actively plays the game as it can result in both getting cooldowns if their the server or the player are found to have committed an infraction.

What do I need to worry about most?
First and foremost all plugins which "..generate/simulate items that are not owned by a player... may result in a GSLT ban.".
As such any plugin which changes a players weapon to one which they may not own should be avoided. This includes vanilla knife mods and other misc weapon mods which give items not in a players inventory.
If players still want to use each others knives the cvar "mp_drop_knife_enable 1" can be set to allow players to safely drop their knives.

I have a community ban, can I still make tokens?
No, currently any account with community bans is unable to use GSLTs.

Ive been banned, what now?
If you do receive a GSLT ban all is not lost, it is still possible to host servers going forward.
You simply need to create new Steam AND game accounts, wait 90 days for the new account period, complete the process of removing the limitations on the new account and then create new tokens.


Addendum
While not currently explicitly stated it is highly likely that using fake spray plugins will also be a bannable offence.
Glove skins were added in a recent update, it is highly likely that fake glove skins are also prohibited.

That question is misleading, you can be market banned (which is not a standalone thing, but instead a part of other ban types) and still create/use GSLTs. The linked part is accurate though, you cannot if community banned.

Due to the recent update adding glove skins it appears highly likely that formerly allowed plugins which allowed players to have different gloves are in violation of the prescribed rules.

They wont get you banned currently, however there's nothing you can do to prevent the new gloves from being over top of the arm model.
