We had the same issue with tag protection and sourcebans, try to add

sb_reload
sm_rehash
sm_reloadadmins
sm plugins reload tagprotection

at the bottom of your server.cfg, this did the trick for me. It looks like sometimes SB dont keep the admins in the memory for some odd reason.
