It is a very hot function so the engine has a lot of optimisations to avoid calling it unnecessarily, it is normally the edict flags
FL_EDICT_FULLCHECK,
FL_EDICT_ALWAYS, and
FL_EDICT_PVSCHECK that get in the way. The core logic is in
CServerGameEnts::CheckTransmit in the SDK if you want to fully understand it.
