1.File name wrong, it should be "v_blackholy.
dx90.vtx"
2.Wrong model name in .mdl, it should be
v_blackholy.mdl, not blackholy.mdl
3.Some spaces you didn't notice in .mdl (for example, it should be 1 space in the end of file.)
4.You put material files in wrong location
