This error appears to be related to the Dodgeball LR. It occurs when the flashbang given to a player instantly disappears. It's not possible for a player to receive the flashbang and throw it that quickly.
So it could be another plugin interfering with these flashbangs. How often does this occur during the Dodgeball LRs? Check what other plugins you have installed that have an OnEntityCreated callback for those are the most likely culprit.
