by the way, I use MM1.10 and SM1.6.0-4241 (due to Left4Downtown2 0.5.7 noplayerslot + L4DToolz)

it happen when going to load next map, kick everyone, sometime load same map with previous score.
add:

sm_cvar z_max_player_zombies 10
sm_cvar survivor_limit 10

to server.cfg and try.
noxman is offline
